Pharmacodynamic studies show that tesamorelin reduces visceral adipose tissue more than subcutaneous adipose tissue in the studied population. This selectivity may relate to differences in blood flow and hormone sensitivity between fat depots. Effects on glucose metabolism and insulin sensitivity have been investigated, with some trials reporting modest changes and others showing stability. The precise relationship between growth hormone exposure, IGF-1 levels, and visceral fat loss remains an active area of analysis.
Tesamorelin binds to growth hormone-releasing hormone receptors on somatotroph cells in the anterior pituitary. Receptor activation increases intracellular cyclic AMP and promotes synthesis and secretion of growth hormone. Because the peptide mimics endogenous GHRH, it amplifies the normal pulsatile release of growth hormone rather than providing exogenous growth hormone directly. This upstream action distinguishes tesamorelin from recombinant growth hormone preparations and from growth hormone secretagogues that act at different receptors.
Stimulated growth hormone release leads to hepatic production of insulin-like growth factor 1, a key mediator of many growth hormone effects. In clinical studies, tesamorelin increased IGF-1 levels in a dose-dependent manner, although the response varies among individuals. The drug's effect on visceral fat is thought to involve growth hormone-mediated lipolysis and altered adipocyte metabolism. Muscle mass and lean body mass have also been assessed as secondary outcomes, but changes are generally smaller and less consistent than fat reductions.

Tesamorelin binds to growth hormone-releasing hormone receptors on the surface of pituitary somatotroph cells. This binding activates adenylate cyclase, raising intracellular cyclic AMP levels and triggering the release of growth hormone into circulation. The elevated growth hormone then stimulates hepatic production of insulin-like growth factor 1. Because the effect is mediated through the endogenous axis, secretion remains subject to feedback regulation. This distinguishes it from direct growth hormone administration, which bypasses pituitary control entirely.
Clinical investigation has focused on HIV-associated lipodystrophy, a condition in which antiretroviral therapy contributes to abnormal fat distribution. Excess visceral adipose tissue accumulates in the abdomen while peripheral fat may be lost. Tesamorelin was evaluated for reducing this visceral fat depot, with trials measuring changes in abdominal fat by imaging rather than by body weight alone. The rationale rests on the known lipolytic effects of growth hormone. Effects on visceral fat are documented, while long-term outcomes regarding cardiovascular risk remain less clearly established.
Tesamorelin binds the growth hormone–releasing hormone receptor on pituitary somatotroph cells. The receptor signals through the Gs protein, raising intracellular cAMP and activating protein kinase A. That cascade triggers release of stored growth hormone in pulses rather than a steady stream. Because the drug acts at the receptor that normally controls this process, its effect depends on the body's own signaling architecture rather than on a synthetic pathway. The resulting hormone profile reflects the timing of each pulse, not only its size.
Measured responses usually involve growth hormone and insulin-like growth factor 1, known as IGF-1. Growth hormone rises in bursts and is difficult to sample reliably, while IGF-1 shifts more slowly and can be assessed from a single blood draw. Studies therefore treat IGF-1 as the more practical pharmacodynamic marker. Both are indirect, showing that the receptor was engaged rather than that the peptide reached a particular concentration. Direct exposure measurement requires an assay aimed at the molecule itself.
==== United Arab Emirates ==== The UAE takes an exceptionally strict line on medicines, with many common drugs, notably anything containing codeine being banned unless one has a notarized and authenticated doctor's prescription. Visitors breaking the rules, even inadvertently, have been deported or imprisoned. The US Embassy to the UAE maintains an unofficial list of what may not be imported.
On 13 February 2007, HGS and GSK announced the initiation of the first of two Phase III clinical trials of belimumab in patients with active lupus erythematosus. Two Phase III clinical studies were conducted, involving a total of 1,684 patients with scores of ≥6 on the SELENA-SLEDAI assessment of lupus activity. The primary end point was a reduction of ≥4 on the SELENA-SLEDAI assessment, and several other factors, after 52 weeks. Belimumab significantly improved the response rate, reduced disease activity and severe flares, and was well tolerated. Among patients treated with belimumab (10mg/kg) in addition to standard therapy, 58% had SELENA-SLEDAI scores reduced by ≥4 points over 52 weeks, compared with 46% of patients treated with placebo. However, patients of African-American or African descent did not respond significantly to belimumab. These trials did not include patients with the most severe forms of systemic lupus erythematosus, which involve active damage to the kidneys or central nervous system. Subjects with active kidney disease were included in Phase II trials. Clinical trials found belimumab to be safe in treating systemic lupus erythematosus, but the magnitude of benefit was small, and Phase III trials excluded the most severe cases of systemic lupus erythematosus, involving kidney and brain damage. Reviewers at the US Food and Drug Administration (FDA) expressed concern that the drug was only "marginally" effective, and that there were more deaths in the treatment group.
When allowed to react with superacids, sulfuric acid can act as a base and can be protonated, forming the [H3SO4]+ ion. Salts of [H3SO4]+ have been prepared (e.g. trihydroxyoxosulfonium hexafluoroantimonate(V) [H3SO4]+[SbF6]−) using the following reaction in liquid HF:
=== Shipanga Affair and PLAN's exit to Angola === In the aftermath of the MPLA's political and military victory, it was recognised as the official government of the new People's Republic of Angola by the European Economic Community and the UN General Assembly. Around May 1976 the MPLA concluded several new agreements with Moscow for broad Soviet-Angolan cooperation in the diplomatic, economic, and military spheres; simultaneously both countries also issued a joint expression of solidarity with the Namibian struggle for independence. Cuba, the Soviet Union, and other Warsaw Pact member states specifically justified their involvement with the Angolan Civil War as a form of proletarian internationalism. This theory placed an emphasis on socialist solidarity between all left-wing revolutionary struggles, and suggested that one purpose of a successful revolution was to likewise ensure the success of another elsewhere. Cuba in particular had thoroughly embraced the concept of internationalism, and one of its foreign policy objectives in Angola was to further the process of national liberation in southern Africa by overthrowing colonial or white minority regimes. Cuban policies with regards to Angola and the conflict in South West Africa thus became inexorably linked. As Cuban military personnel had begun to make their appearance in Angola in increasing numbers, they also arrived in Zambia to help train PLAN.
multicellular Composed of more than one cell. The term is used especially to describe organisms or tissues consisting of many cells descendant from the same original parent cell which work together in an organized way, but may also describe groups of nominally single-celled organisms such as protists and bacteria which live symbiotically with each other in large colonies. Contrast unicellular.

Myeloproliferative disease (primarily essential thrombocythemia and polycythemia vera). It has been found to be superior to anagrelide for the control of ET. Sickle-cell disease (increases production of fetal hemoglobin that then interferes with the hemoglobin polymerisation as well as by reducing white blood cells that contribute to the general inflammatory state in sickle cell patients.) Second line treatment for psoriasis (slows down the rapid division of skin cells) Systemic mastocytosis with associated hematological neoplasm(SM-AHN) (The utility in treating SM-AHN with hydroxycarbamide stems from its myelosuppressive activity, it does not however exhibit any selective anti-mast cell activity) Chronic myelogenous leukemia (largely replaced by imatinib, but still in use for its cost-effectiveness)
Russia claimed to have taken the village of Keramik in Donetsk Oblast. Ukraine claimed to have shot down a Russian Su-25 fighter jet over Donetsk Oblast. Two people were killed by Russian bombing in Vovchansk. One person was killed in a missile strike in Sumy. In Russia, drones were reported over Belgorod and Kursk Oblasts. Several projectiles were shot down but debris left "several dozen" private homes, mostly smashed windows, and cars damaged. Russian officials subsequently claimed that five people were killed and nine injured by drone strikes in Belgorod, Kursk and Donetsk Oblasts. A Russian Iskander missile struck a Ukrainian forward arming and refuelling point in Pavlohrad, Donetsk Oblast. Two Mi-24s and a Mi-17 helicopter which had just landed were seriously damaged. Video was released of a Russian drone striking a Ukrainian Buk-M1 missile system which appeared to have been fitted with US made RIM-7 Sea Sparrow missiles instead of the original 9M38 missiles.

== Interactions == Pindolol has been found to robustly potentiate the effects of the serotonergic psychedelic dimethyltryptamine (DMT). This is thought to be due to its serotonin 5-HT1A receptor antagonism, which is thought to disinhibit the effects of DMT.

== Health policy == Obesity is a complex public health and policy problem because of its prevalence, costs, and health effects. As such, managing it requires changes in the wider societal context and effort by communities, local authorities, and governments. Public health efforts seek to understand and correct the environmental factors responsible for the increasing prevalence of obesity in the population. Solutions look at changing the factors that cause excess food energy consumption and inhibit physical activity. Efforts include federally reimbursed meal programs in schools, limiting direct junk food marketing to children, and decreasing access to sugar-sweetened beverages in schools. The World Health Organization recommends the taxing of sugary drinks. When constructing urban environments, efforts have been made to increase access to parks and to develop pedestrian routes. Mass media campaigns seem to have limited effectiveness in changing behaviors that influence obesity. At the same time they can increase knowledge and awareness regarding physical activity and diet, which might lead to changes in the long term. Campaigns might also be able to reduce the amount of time spent sitting or lying down and positively affect the intention to be active physically. Nutritional labelling with energy information on menus might be able to help reducing energy intake while dining in restaurants.
Chelation therapy is a medical procedure that involves the administration of chelating agents to remove or deactivate heavy metals from the body. Chelating agents are molecules that form particularly stable coordination complexes with metal ions. Complexation prevents the metal ions from reacting with molecules in the body, and enable them to be dissolved in blood and eliminated in urine.
For instance, whereas the AR activity of testosterone is greatly potentiated by local conversion via 5α-reductase into DHT in tissues where 5α-reductase is expressed, an AAS that is not metabolized by 5α-reductase or is already 5α-reduced, such as DHT itself or a derivative (like mesterolone or drostanolone), would not undergo such potentiation in said tissues. Moreover, nandrolone is metabolized by 5α-reductase, but unlike the case of testosterone and DHT, the 5α-reduced metabolite of nandrolone has much lower affinity for the AR than does nandrolone itself, and this results in reduced AR activation in 5α-reductase-expressing tissues. As so-called "androgenic" tissues such as skin/hair follicles and male reproductive tissues are high in 5α-reductase expression, while skeletal muscle is virtually devoid of 5α-reductase, this may primarily explain the high myotrophic–androgenic ratio and dissociation seen with nandrolone, as well as with various other AAS. Aside from 5α-reductase, aromatase may inactivate testosterone signaling in skeletal muscle and adipose tissue, so AAS that lack aromatase affinity, in addition to being free of the potential side effect of gynecomastia, might be expected to have a higher myotrophic–androgenic ratio in comparison. In addition, DHT is inactivated by high activity of 3α-HSD in skeletal muscle (and cardiac tissue), and AAS that lack affinity for 3α-HSD could similarly be expected to have a higher myotrophic–androgenic ratio (although perhaps also increased long-term cardiovascular risks).
